Monitoring Bone Density in Hypogonadal Men by Quantitative Phalangeal Ultrasound. Therefore, we assessed the feasibility of a more practical and economic approach applying phalangeal quantitative ultrasound pQUS, IGEA DBM BP sonic 1200, Sensweiler, Germany ; in a cross-sectional study in 521 men aged 18 - 91 years 224 healthy controls, 156 newly diagnosed hypogonadal and 141 T substituted men ; . The method was compared to QCT of the lumbar spine in the first 80 patients. We evaluated longitudinal changes of amplitude-dependent speed of sound AdSoS ; in 54 hypogonadal men from the beginning of T substitution. AdSoS decreased with age p 0.0001 ; and increased 4-5 fold per 1 nmol L increment of total T in the hypogonadal range 12 nmol l ; compared to the eugonadal range p 0.0001 ; . AdSoS was higher in eugonadal and substituted men than in hypogonadal patients p 0.0001, ANCOVA ; . Substituted men younger than 50 years showed lower AdSoS than eugonadal men p 0.004 ; and untreated men with secondary hypogonadism had lower values than men with primary hypogonadism p 0.005 ; . Therapeutic effects were seen regardless of age, diagnosis or T substitution modality. In the longitudinal approach, AdSoS increased from 1986 93 to 2035 77 m s over 237 57 days with the highest gain seen in those men with initially lowest values p 0.0001, ANCOVA for repeated measurements ; . It is the power that this salt has to use water, that renders it of so much value to animal and vegetable life. When we take too much salt with our food we are continually drinking water to preserve the proportion the superabundance of salt in the system demands; and, again, when there is too little chloride of sodium, the fluids within the body call for it in many ways. It makes its appearance in many different channels, as in watery pimples or blisters, frothy, watery bubbles on the tongue, excessive flow of tears, excessive flow of saliva, watery secretions from the nose, vomiting of this watery fluid, waterbrash not acid, and in any condition where there is an excess of watery exudate. At one time especially in the South, chloride of sodium was used in the treatment of ague, and in many cases with marked success where quinine and many other well known antiperiodics had failed. I presume that many of my readers will at once take issue with me when I say that in ague and senna.

Clinical assessment of heart failure HF ; patients, based only on physical exam, has been shown inappropriate, both for differential diagnosis of preserved or not systolic function1, 2, and to estimate hemodynamic condition of such patients 3. At a time prior to the use of betablockers, mortality among this group was estimated in 36% in 1 year, such as in CONSENSUS4 study, and in 37% in 6 months, in FIRST5 study. At beta-blocker use time, yearly mortality, observed in COPERNICUS6 study, was 11.4% in carvedilol-treated patients and 18.5%, in those treated with placebo. Heart transplantation is the best treatment available for that population, but it finds limited application due to low offer of donors. Alternative treatments have been more and more studied, aiming at the improvement of quality and quantity of life of patients. Surgery of DOr, ventricular resynchronization therapy7, 8 and stem cell implant in the myocardium9, among others, are examples of those treatments. The use of invasive hemodynamic monitoring IHM ; , with Swan-Ganz catheter to direct advanced HF pharmacological treatment, has been used in the last years with promising results10-12. In most cases, ventricular filling pressures can be significantly reduced with that treatment, as well as separate subgroups of lower severity that do not respond to the treatment, improve symptoms and remove patients from implant lines13. This study aimed at using the strategy of treating advanced HF patients, based on data obtained through IHM, and observing whether is possible to reduce ventricular filling pressures significantly, by using high doses of vasodilators and diuretics. Consequences of that strategy in laboratorial parameters of renal and electrolytic function were also assessed and senna.

This catheter also served as a site to obtain arterial blood samples that were analyzed for pH, Po2 and Pco2 on an IL-813 automated blood gas analyzer. Ventilation was adjusted throughout the experiment to maintain arterial Po2 at a level greater than 85 mm Hg. Sodium bicarbonate was infused intravenously as needed to correct for base deficit and maintain arterial pH within 7.35-7.47. A left thoracotomy at the fifth intercostal space was performed and the lungs were gently retracted. To produce neural decentralization of the heart, 9 10 the ansae subclaviae were isolated as they exited from the stellate ganglia, doubly ligated and transected. The cervical vagi were doubly ligated and transected at this time. Miniature shielded bipolar stimulating electrodes Ealing Manufacturing Co. ; were positioned on the left and right ansae subclaviae to stimulate efferent sympathetic nerves to the heart. Stimuli were square-wave pulses 4 msec, 7-9 V, 2-4 mA ; delivered at a rate of 3-5 Hz from a constant current source. This frequency has been described as the "tonic" rate of activity"' 12 for the cardiac sympathetic nerves and produces shortening of the effective refractory period ERP ; without excessively increasing heart rate. The pericardium was opened, 5-7 mm of the first diagonal branch of the left anterior descending coronary artery LAD ; were isolated, and two silk ligatures were placed underneath the artery. Because isolation of the coronary artery might interrupt sympathetic fibers traveling in the adventitia, after coronary artery dissection, refractory period determinations were performed before and during stellate stimulation to ensure that all areas tested were not denervated by the coronary artery isolation. All sites tested in this study had ERP shortening during left and right stellate stimulation after the coronary artery isolation. Endocardial and epicardial bipolar plunge electrodes, constructed of two Teflon-coated stainless steel wires threaded through a 21-gauge needle and bent at the tip to form a hook, were positioned above basal ; and below apical ; the zone that potentially would be made ischemic by the latex injection of the artery. Each endocardial electrode was placed beneath an epicardial electrode. The electrode positions, area of latex injection, and the area of potential sympathetic denervation are shown in figure 1.
