International experience may provoke animosity towards those returning home Editor--Lucas's editorial once again raises the important issue of migration of healthcare professionals from Africa to the West.1 However, this is a general issue faced by all developing nations and includes internal migration in Africa from poorer to wealthier African countries.25 The way in which healthcare professionals returning to their home country from a period of working or training abroad are received may be an important consideration. They are often welcomed back, and the experience gained abroad will be shared with colleagues. However, there may occasionally be animosity from those who have not gained international experience to those who have. The returning healthcare professional may find it difficult to adjust to working in a developing nation with restriction on facilities, investigations, and drugs. The impact of this on their personal job satisfaction is not determined. Furthermore, the international experience gained will include experience of personnel departments and conditions of service that value the healthcare professional. In my experience the value placed on public sector healthcare professionals in the developing world is minimal, and this can be very frustrating.

Analysis of 47 cells showed that 100 percent had male karyotypes and no Philadelphia chromosome. At this writing the patient is well and has returned to work.

The renal Na : Cl cotransporter rNCC is mutated in human disease, is the therapeutic target of thiazide-type diuretics, and is clearly involved in arterial blood pressure regulation. rNCC belongs to an electroneutral cation-coupled chloride cotransporter family SLC12A ; that has two major branches with inverse physiological functions and regulation: sodium-driven cotransporters NCC and NKCC1 2 ; that mediate cellular Cl influx are activated by phosphorylation, whereas potassium-driven cotransporters KCCs ; that mediate cellular Cl efflux are activated by dephosphorylation. A cluster of three threonine residues at the amino-terminal domain has been implicated in the regulation of NKCC1 2 by intracellular chloride, cell volume, vasopressin, and WNK STE-20 kinases. Nothing is known, however, about rNCC regulatory mechanisms. By using rNCC heterologous expression in Xenopus laevis oocytes, here we show that two independent intracellular chloride-depleting strategies increased rNCC activity by 3-fold. The effect of both strategies was synergistic and dose-dependent. Confocal microscopy of enhanced green fluorescent protein-tagged rNCC showed no changes in rNCC cell surface expression, whereas immunoblot analysis, using the R5-anti-NKCC1-phosphoantibody, revealed increased phosphorylation of rNCC amino-terminal domain threonine residues Thr53 and Thr58. Elimination of these threonines together with serine residue Ser71 completely prevented rNCC response to intracellular chloride depletion. We conclude that rNCC is activated by a mechanism that involves amino-terminal domain phosphorylation.
The new and youngest additions to the professorial stable at ages thirty-five and thirty-six. Actually, in 1959 Barry Mazur, Shlomo Sternberg, and David Mumford were also already on board as Junior Fellows and assistant professors. I was of course scared to death to join a department that included such names as Ahlfors, Brauer, Zariski, and I tried hard to be on best behavior. The department was extremely hospitable; we were wined and dined, and soon learned that nobody would bite us. Still, it was only after a delightful evening at the Ahlforses' that Phyl and I truly felt at ease here. Somehow, in their house, matters of protocol or hierarchy--as well as being on one's good behavior--were completely missing. We were there to have a good time and to bare our souls to each other, helped along with the obligatory Skol in vodka, often followed by champagne. In the department Lars was a quiet, reserved, and benevolent presence. He was always perfectly attired, attended the meetings punctually, and only now and then entered the discussions, often decisively. For me it was always a pleasure to run into him in the corridors. It reassured me: this was indeed a good place to be. At home Lars was the warmest of hosts, a delightful drinking companion, a devotee of beauty and class in every form--from literature, music, and painting to the admiration of the opposite sex. Yet there was no impropriety in Lars's gallantry. All his life he was enthralled with his wonderful wife, Erna, who had such a mercurial way of dealing with his natural Finnish reserve and his shyness. I now remember with special affection a summer month we spent together with the Ahlfors family on the coast of Rhode Island, with the many minor misadventures that invariably occur in a joint rental venture. For instance, each family had a dog, both male, who unfortunately would not tolerate each other. Amazingly, this did the job; "out of sight, out of mind" really works for dogs. Actually, on second thought and on observing my grandchildren, maybe four kids under five years of age is an even match for one child over fourteen. In any case, it was a memorable month, with Erna and Phyl outdoing each other with their consummate culinary skills. Lars was a great Harvard patriot.
